This theory-based module aims to support participants to critically analyse and evaluate the principles and theories of optimal care management of persons living with and dying from chronic and/or advancing heart failure. A range of evidenced based topics/themes applicable at a national and international level will be utilised throughout. Participants will critically explore the causes and effects of heart failure and will have the opportunity to evaluate current diagnostic, assessment, planning and interventional strategies and frameworks to encourage gold standard care. Participants will regard patients and their significant others as genuine partners throughout this whole process and will work together in seeking the best outcome for each individual.

This module enables participants to blend key heart failure and palliative care concepts through analysis, synthesis and critical appraisal. It considers the wider organisational drivers as well as the challenges presented to professionals working with diversity.
Heart Failure Management in Clinical Practice & MSc in Advancing professional practice (heart failure practice – here
The standalone module or full MSc programme is suitable for health care professionals working in the field of ambulatory heart failure, including General Practice Nurses and Advanced Practitioners who manage long term condition clinics and GPNs with a special interest; allied health professionals involved in heart failure rehabilitation and primary and secondary care heart failure specialist nurses. This programme will develop practitioners as HF specialists, fully equipped to meet the assessment and management challenges that patients with HF often present
