Key UK Heart Failure Statistics

Heart failure is a major driver of NHS winter pressures and emergency bed occupancy, yet it remains one of the most preventable causes of emergency hospital admissions.

  • 1 Million+ people in the UK living with heart failure

  • 385,000 undetected and undiagnosed cases missing out on life-preserving care

  • 80% of heart failure cases are diagnosed in hospital, despite 40% showing symptoms in primary care months prior

  • 2% of the total NHS budget spent on heart failure management

Complex heart failure challenges cannot be solved in isolation; BSH actively partners with parliamentarians, integrated health and care systems and charity networks to bring about change.

Our Calls to Action

Parliamentarians & Policymakers

Champion Heart Failure in Parliament

  • Drive Strategic Priority: Advocate for heart failure to be integrated into broader cardiovascular and chronic disease national strategies.

  • Table Parliamentary Questions: Highlight regional health inequalities and urge local healthcare systems to publish early diagnosis metrics.

  • Join the All-Party Parliamentary Group (APPG): Support legislation that expands the specialist heart failure workforce and protects community specialist nurse provision.

Healthcare Systems and Organisations

Transform Local Service Delivery

  • Embed Early Diagnosis Pathways: Implement rapid blood testing (NT-proBNP) and diagnostic pathways in primary care to catch heart failure before emergency admission.

  • Invest in Community Care: Expand Heart Failure Specialist Nurse teams and community-based virtual wards to reduce avoidable hospital bed days.

  • Target Local Health Inequalities: Use BSH national audit data to address local care gaps and ensure equal access to specialist therapies regardless of postcode.

Charity and Third Sector

Unite for Collective Impact

  • Join our Collaborative: Partner with over 45 health organisations working in unison to reduce heart failure mortality.

  • Co-Design Campaigns: Collaborate on national awareness campaigns (such as World Heart Day) to educate the public on early warning signs.

  • Empower Patients & Carers: Share BSH-accredited patient resources, guidance frameworks, and self-management tools across your networks.

Our Core Policy Pillars

1. Early Detection, Diagnosis & Prevention

  • The Strategy: Shift management upstream by establishing national standards for early diagnosis, ensuring universal primary care access to blood testing ($NT-proBNP$), and timely echocardiography.

  • The Impact: Hospitalisation accounts for 60–70% of all heart failure costs; diagnosing patients earlier prevents emergency crisis admissions, saves thousands of lives, and yields massive NHS cost savings.

2. National Model Service & Workforce Plan

  • The Strategy: Publish a BSH Model Service Specification explicitly defining mandatory Multi-Disciplinary Team (MDT) composition, staffing ratios, and service standards across inpatient, outpatient, and community care.

  • The Impact: Patients admitted under specialist care are nearly half as likely to die in hospital; reducing emergency admissions by 10–20% releases over £100m back to the NHS while relieving severe winter pressures.

3. Equitable Access to Optimal Treatment

  • The Strategy: Ensure every patient—regardless of geography—has rapid access to full evidence-based, four-pillar therapies and multimorbidity-aligned care across the entire ejection fraction spectrum ($HFpEF$, $HFmrEF$, $HFrEF$).

  • The Impact: Rapid guideline-directed medical therapy (GDMT) optimization drastically cuts 90-day rehospitalisations and addresses severe regional health inequalities.

4. Digital, Data-Driven & Integrated Pathways

  • The Strategy: Accelerate the adoption of AI-enabled risk stratification, biomarker-driven triage, and remote monitoring standards to integrate primary, secondary, and virtual ward care.

  • The Impact: With 20% of patients readmitted within a year, smart integrated monitoring reduces administrative load, prevents avoidable readmissions, and saves millions annually.

5. Continuous Quality Improvement & Research

  • The Strategy: Embed national audits, implementation science, and structured quality improvement programmes to rapidly translate research into routine frontline clinical practice.

  • The Impact: Five-year heart failure survival remains worse than many common cancers; eliminating regional variation saves lives immediately without requiring new drug expenditure.
